CMI participates with most insurance companies. If CMI does not participate with your insurance, you can still have your MRI with us but you may have a balance. Contact your insurance to find out what your responsibility for out of network testing is. Most insurance companies require pre-authorization for an MRI. It is the responsibility of the ordering provider to obtain this authorization. This is not always a guarantee of payment. All insurance companies vary. Contact your insurance carrier if you have questions about what your responsibilities of payment are. Below is a list of the insurance companies that we currently participate with.
Please Note: It is recommended that you bring any health care provider forms and/or information so that we may best assist you. This will include all compensation and/or no fault information. You may need to obtain and bring with you all prior CT scans and X-rays pertaining to your present condition. Please ask us at the time of scheduling your MRI if this is necessary. Many insurance plans require co-payments at the time of service as part of your contract. We expect that you are familiar with your insurance coverage and co-pay requirements. We accept cash, check and most major credit cards at our campuses.
You may also inquire about account balances and make payments to your account at time of registration.

For patients that do not have insurance, CMI requires a down payment at the time of service and payment arrangements may be set up with our billing office. This payment can be paid in cash, check or credit card. Please contact our main office for more information.
For Workers Compensation studies it is not necessary to obtain authorization for certain exams, but we will verify coverage. This means that verification of each body area needs to be made before the patient can be scanned. A lot of carriers use third party networks. CMI provides these networks with all of the Workers Compensation information that they require and the billing will be processed through their service.
Patients with no-fault insurance must have all no-fault information and complete a form at the time of appointment.
